Thank you for requesting a digital (PDF) version of the Sleepwell booklet(s). Please note that digital copies are not available for personal use or for sharing with others for any reason. We only share our digital versions when requested for:

1) local professional printing and distribution to support access to Sleepwell’s printed booklets by health care professionals, patients, and the public; or
2) collaborations with us involving research, booklet translation, or both.

Please take a few moments to complete this form. Once submitted, we will review your request and contact you by email. If your request is approved, you will be invited to complete our standard licensing agreement. It is customary for a fee to be included with each agreement.

Current options:

Explain why the current options (large orders and small orders) are not suitable for your needs. For example, your use is outside of Canada and the US, or you will use your organization’s professional printing and binding infrastructure for full-colour, saddle-stitched printing. These limitations are in place to ensure that the booklets are shared in the format that aligns with the evidence of their effectiveness.
